New Customer Information and Maintenance Authorization Form

Please complete the following form so we may have your information on file. FlightLogix LLC may require the following to begin maintenance on any aircraft:

• This completed form
• A signed purchase order on file
• Logbooks and other documentation

Please submit all signed purchase orders to maintenance@flightlogix2.rowebdev.com or fax to (513) 321-1842.

All invoices are due upon receipt. A deposit may be required for first-time customers or substantial work orders. Labor rates are posed on our website and are subject to change. Maintenance is subject to a miscellaneous consumables charge and applicable taxes (There is NO sales tax in Ohio on aircraft parts!) There will be a 1% charge for City of Cincinnati Airport Tax. All parts are subject to freight charges.

By submitting this form, you certify that you are authorized to make maintenance decisions on the listed aicraft and authorize FlightLogix LLC or a third party vendor authorized by FlightLogix LLC to perform maintenance including acquisition of parts, fluids, and other services on said aircraft.

Once you submit this form, you will be emailed a credit card authorization form. We must have this form and the credit card authorization form before we can begin maintenance on your aircraft.
